About Foundation for Safe Transitions
The Foundation for Safe Transitions is a dedicated nonprofit committed to ending homelessness among veterans, seniors, and young adults. We believe that everyone deserves a safe place to call home and the opportunity to rebuild their lives with dignity and respect. Our mission is rooted in the understanding that stable housing is the foundation for personal growth, independence, and community integration.
We provide comprehensive support services that go beyond shelter, including case management, job training, mental health resources, and VA-aligned partnerships. Our programs are designed to foster stability, dignity, and long-term independence through community collaboration and evidence-based practices. By combining safe transitional housing with wraparound services, we create pathways that empower individuals to achieve permanent housing and sustainable independence.
Our Services
Foundation for Safe Transitions is proud to announce the launch of six new transitional housing beds, expanding our capacity to serve veterans, elderly individuals, and young adults experiencing homelessness. As a 501(c)(3) nonprofit, we work closely with VA-aligned referral partners, community organizations, and dedicated case managers to ensure coordinated, individualized support for every participant. Our transitional housing program is intentionally designed with a clear focus on permanent housing exits—helping individuals move from crisis to long-term stability with dignity. Through strong community collaboration, intensive case coordination, and proven pathways to self-sufficiency, we are working to end veteran, elderly, and young adult homelessness by providing safe transitional housing and supportive services that restore stability and hope.
